About the Role

The Intellectual/Developmental Disabilities (I/DD) Services Coordinator works under the direction of the I/DD Program Manager, providing coordination of services for persons experiencing intellectual and/or developmental disabilities. The role involves assessing needs, facilitating services planning, making referrals, and authorizing services for individuals with intellectual and developmental disabilities, while collaborating closely with program participants, guardians, families, and community partners.

Responsibilities

  • Conducts assessments, level of care planning, case management contacts, monitoring, eligibility and transition documentation, and other requirements in compliance with OARs and organizational standards.
  • Completes timely clinical documentation, including progress notes, reports, correspondence, and behavioral data to justify program billing and meet agency requirements.
  • Coordinates and follows up with social service agencies, medical providers, and allied agencies to ensure continuity of care and an integrated service system.
  • Provides case management services to eligible individuals in accordance with OAR 411-415 in various community settings, prioritizing client preference.
  • Attends required case manager trainings, including completion of online core competency modules within the first 90 days of employment and 20 hours of annual training.
  • Facilitates or participates in annual assessments (e.g., CNA, ANA, SNAP, SIS) to determine service needs and funding.
  • Facilitates annual Oregon Needs Assessments (ONA) to determine support needs, risks, and Level of Care (LOC) approval for Medicaid-funded services.
  • Ensures individual choice of case management, provider services, and service settings annually, and provides notification of rights.
  • Coordinates service planning, including completion of annual Individual Support Plans (ISP) and Annual Plans (AP).
  • Assists clients in acquiring home and community-based services and resources to achieve their goals and maintain independence.
  • Makes referrals to independent and agency providers, medical or behavioral health providers, employment services, and OVRS as requested.
  • Collaborates with families, community partners, residential providers, and other social service agencies.
  • Participates in entry and exit meetings, including transition planning and documentation for successful transitions.
  • Reviews provider progress notes, PSW time sheets, positive behavior support plans, and other documentation to ensure funded services and payment of claims.
  • Attends IEPs and transition meetings for individuals and assists with transition planning for those moving to adult services.
  • Ensures timely incident reporting by providers, families, and individuals, and documents serious events in the centralized reporting database.
  • Completes required training in the Centralized Abuse Management (CAM) reporting system.
  • Reports abuse of individuals with intellectual or developmental disabilities and coordinates with protective services, law enforcement, and other agencies.
  • Assists individuals in obtaining Medicaid services and OHP eligibility, and coordinates redetermination of eligibility as required.
  • Assists the Designated Referral Contact in coordinating residential placements for individuals as needed.
  • Ensures service quality and compliance with applicable OARs, maintaining documentation in electronic and file room records.
  • Acts independently in the community, promoting a positive image of Crook County and the I/DD Program.
  • Serves as a liaison between the agency and community or other County personnel, providing technical information, referrals, and training.
  • Performs other related duties as assigned.

Requirements

  • Bachelor’s degree in behavioral science, social science, or a closely related field or Bachelor’s degree in any field plus one year of human services-related experience or Associate’s degree in behavioral science, social science, or a closely related field plus two years of human services-related experience or at least three years of human services-related experience.
  • Experience working with individuals with intellectual or developmental disabilities.
  • For individuals recovering from addiction, must have maintained continuous sobriety for the previous two years at the time of hire.
  • Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization’s auto liability coverage policy (minimum 21 years of age with no Type A violations in the past 3 years or three or more Type B violations).

Preferred Qualifications

  • Bachelor’s degree in behavioral science, social science, or a closely related field.
  • Bilingual in English/Spanish.

Skills

  • Knowledge of the public system for Developmental Disability Services in Oregon and familiarity with Oregon Administrative Rules governing services to persons with intellectual/developmental disabilities.
  • Working knowledge of the public service system for adults and children with intellectual or developmental disabilities in Oregon.
  • Ability to work effectively and respectfully in a diverse, multi-cultural environment.
  • Demonstrated skills in developing cooperative and collaborative efforts with providers, families, advocates, and persons with developmental disabilities.
  • Strong interpersonal and customer service skills, including the ability to assist and advocate for program participants in group and individual settings.
  • Proficiency in MS Office 365 (Word, Excel, Outlook), EHR platforms, databases, virtual meeting platforms, and the internet, with the ability to learn new or updated software.
  • Strong communication skills (oral and written).
  • Strong organizational skills, attention to detail, accuracy, and follow-through.
  • Excellent time management skills with a proven ability to meet deadlines.
  • Critical thinking skills and the ability to maintain strict confidentiality as required by HIPAA, 42 CFR, and Oregon Statutes.
  • Ability to build and maintain positive relationships and function well in a high-paced, sometimes stressful environment.
  • Ability to manage conflict resolution and respond appropriately to anger, fear, hostility, or violence.
  • Ability to work independently as well as participate as a positive, collaborative team member.
