Hybrid Nurse Practitioner - Part Time

About the role

Provide comprehensive primary care for high-needs geriatric patients in home-based settings and a practice location. The position is part-time with 20 hours of clinical work and 4 hours of administrative work (remote possible for administrative tasks).

Responsibilities

• Evaluate and treat medically complex, homebound older adults
• Coordinate care with team members and specialists
• Participate in interdisciplinary meetings
• Perform patient assessments and develop care plans
• Provide night call coverage
• Utilize a productivity model with bonus potential
• Support a multidisciplinary team with dedicated medical assistant support

Requirements

• Nurse Practitioner licensure in Maryland
• CPR certification
• Reliable transportation

Qualifications

Preferred experience includes:

• 3+ years in primary, urgent, or acute care
• Additional certifications in geriatrics or palliative care
