Part-time Nurse Practitioner

We are seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equitable, value-based primary care to adult Medicare patients. Key responsibilities include: providing exceptional primary care, conducting office visits, administering wellness visits, coordinating care with specialists, leading population health initiatives, assisting with triage and outreach, educating patients, conducting home visits, and supporting promotional activities. The role involves working within a collaborative outpatient care team, reporting to the Center Medical Director, and working closely with operational leadership. Qualifications: Master’s or Doctorate in Nursing, national certification in Family, Adult-Gerontology, or Gerontological Nurse Practice, active state license, DEA license, and US work authorization. Preferred skills: primary care experience, knowledge of HCC documentation, ICD-10 coding, population health, teamwork, and bilingual abilities. Details: Part-time, 40 hours/week, with a competitive pay range of $87,035 - $187,460. Benefits include health, dental, vision, wellness resources, and other support programs.
