Part-time Nurse Practitioner

Role: Nurse Practitioner, Advanced Practice Provider Responsibilities: Provide effective, equitable, value-based primary care to adult Medicare patients, conducting routine and acute office visits, annual wellness assessments, and health risk evaluations. Collaborate with a multidisciplinary care team to ensure care coordination, participate in population health initiatives, educate patients, and assist with outreach and phone triage. May conduct home visits and support promotional activities, working under the supervision of the Center Medical Director. Qualifications: Master’s or Doctorate in Nursing, national certification in Family, Adult-Gerontology, or Gerontological Nurse Practitioners, active state NP license, DEA license, and US work authorization. Preferred: primary care experience, familiarity with HCC, ICD-10 coding, and population health strategies; bilingual skills are a plus. Skills & Qualities: Strong teamwork, patient relationship-building, holistic health assessment, documentation accuracy, and commitment to innovative, population-focused care models. Ability to work independently in outpatient settings and coordinate with specialists.
