About the role

Seeking a dedicated Human Service Professional to provide trauma-informed, recovery-focused services to individuals experiencing mental health challenges, homelessness, or involvement with the criminal justice system. This role involves delivering crisis support, case management, and resource navigation in various settings such as community locations, shelters, jails, offices, and schools, either in person or via digital platforms.

Responsibilities

• Assist with Medicaid, FoodShare, Social Security, housing, employment, medication management, and education applications
• Collaborate with clients and teams to promote independence and stability through evidence-based practices
• Maintain accurate documentation of services provided

Qualifications

• Bachelor’s degree in a human services-related field
• Valid driver’s license and reliable transportation
• Strong communication and organizational skills
• Comfort working with individuals in crisis
• Proficiency with electronic health records
