About the role

Seeking a dedicated Human Service Professional to provide trauma-informed, recovery-focused services to individuals facing mental health challenges, homelessness, or involvement with the criminal justice system.

Responsibilities

• Support clients across diverse settings such as community locations, shelters, jails, schools, and offices, through in-person or virtual interactions
• Assist with Medicaid and FoodShare enrollment
• Assist with Social Security, ID, employment, and housing applications
• Provide mental health coordination and recovery support
• Teach life skills, coping strategies, and budgeting
• Support medication management
• Assist with educational planning (GED, college)
• Provide community services support

Requirements

• Bachelor’s degree in a human services field
• Valid driver’s license
• Strong communication, documentation, and organizational skills
• Comfort working in community-based environments
• Proficiency with electronic health records
