Job Summary We are seeking a Human Service Professional to provide trauma-informed, recovery-focused services to individuals experiencing mental health challenges, homelessness, or involvement with the criminal justice system. In this role, you will deliver support across various settings such as community locations, shelters, jails, offices, and schools, utilizing in-person and virtual methods. Key responsibilities include assisting with Medicaid/FoodShare enrollment, social security, IDs, employment, housing applications, and offering mental health support, life skills, medication management, and educational coordination. You will collaborate with clients and teams, maintain accurate documentation, apply evidence-based practices, and promote client independence. Qualifications: Bachelor's degree in a human services-related field is required, along with a valid driver’s license, reliable transportation, and strong communication and organizational skills. Ability to work in community environments and use electronic health records is essential.
