Human Services Professional

About the role

We are seeking a Human Service Professional to provide trauma-informed, recovery-focused services to individuals experiencing mental health challenges, homelessness, or involvement with the criminal justice system. In this role, you will deliver support across various settings such as community locations, shelters, jails, offices, and schools, utilizing in-person and virtual methods.

Responsibilities

• Assist with Medicaid/FoodShare enrollment, social security, IDs, employment, and housing applications
• Provide mental health support, life skills, medication management, and educational coordination
• Collaborate with clients and teams
• Maintain accurate documentation
• Apply evidence-based practices
• Promote client independence

Qualifications

• Bachelor's degree in a human services-related field
• Valid driver’s license and reliable transportation
• Strong communication and organizational skills
• Ability to work in community environments
• Experience using electronic health records
