Housing Support Mental Health Practitioner/Children's Case Manager

About the role

Fernbrook Family Center is a community mental health agency seeking a Case Manager / Mental Health Practitioner (MHP) to provide targeted mental health case management and rehabilitative services to children, adults, and families.

Responsibilities

• Provide targeted mental health case management services to children, adults, and families.
• Deliver home and community-based mental health skills services under the supervision of a Qualified Mental Health Professional (QMHP).
• Care for clients by coordinating referrals and collaborating with medical providers, schools, county agencies, and community resources.
• Build and maintain strong working relationships with clients, families, and community partners.
• Maintain face-to-face contact with clients based on clinical need, and attend quarterly treatment team meetings.
• Support clients in achieving their treatment goals and overcoming barriers to success.
• Assist with discharge planning and transitions from hospitals, residential treatment, correctional facilities, and other placements.
• Maintain timely, accurate, and high-quality clinical documentation.
• Participate in supervision, staff meetings, and ongoing professional development.
• Complete 30 hours of continuing education related to mental health every two years.

• Follow all agency policies, procedures, and documentation requirements.
• Perform other duties as assigned.

• Candidates must meet Minnesota Mental Health Case Manager qualifications, including a Bachelor's degree in a behavioral science or related field, or three to four years of experience as a Case Manager Associate, or Registered Nurse with specialized psychiatric training and three years of qualifying mental health experience, or 4,000 supervised hours providing mental health services, or 30 semester hours in behavioral sciences plus 2,000 supervised hours providing mental health services.
• In addition, candidates must have reliable transportation, a valid driver's license and acceptable driving record, current automobile insurance, and the ability to pass required criminal background studies in accordance with Department of Human Services requirements.
• Preferred qualifications include experience providing services to children, adults, and families, previous targeted case management experience, experience delivering mental health rehabilitation or skills services, knowledge of community resources and systems of care, and bilingual or multilingual abilities.
