Housing Stabilization Case Manager $2500 retention bonus

About the Role

The Housing Stabilization Case Manager provides housing case management support for the Providence Center’s housing department. They deliver housing stabilization services to participants, complete filing and documentation, develop and use spreadsheets in Excel, perform data entry in EPIC, and coordinate referrals for supportive services.

Responsibilities

• Perform a variety of duties to support consumers and staff in the department.
• Collaborate with supervisor on scheduling interactions with recipients to meet monthly target measures.
• Work with staff to open Housing Episodes in Epic to ensure participants have a medical record number and documentation of services provided.
• Respond to incoming telephone calls from internal/outside agencies, insurance companies, staff, and consumers; refer to appropriate staff, service component, or community resource.
• Answer calls courteously, transfer calls appropriately, and ensure messages are accurately recorded.
• Answer questions and investigate until resolution if the answer is not immediately known.
• Manage data in group folders to track target measures.
• Complete intakes and treatment plans for Housing Stabilization services.
• Provide Housing Stabilization services to participants.
• Update census and track hours of service for each participant.
• Complete quick notes for any services provided directly to or on behalf of tenants.
• Follow up with outside providers of program participants to track engagement in services; ensure releases are completed and valid.
• Schedule intakes for new participants and assist with referrals to outside agencies as needed.
• Attend HSP meetings to discuss workflow, targets, and billing.
• Work with clinical programs to identify services needed for recipients.
• Copy, scan, fax materials, and file charts and notes as needed.
• Work with EOHHS on prior authorizations, correspondence, and required data for tracking.
• Provide direct alcohol and drug counseling/case management to individuals with substance use disorders.
• Maintain cooperative relationships with employees, consumers, and the public; treat consumers and peers with respect.
• Work cooperatively with co-workers and supervisory staff; address problems constructively.
• Accept and use direction and supervision.
• Attend trainings, conferences, and center committees; provide feedback on educational programs attended.
• Attend mandatory in-service trainings and other required trainings.

Requirements

• Bachelor's Degree required; Human Services field preferred.
• Minimum 6 months to 1 year of housing or case management experience.
• Ability to work with a diverse population and comfortable interacting with clients and outside agencies.
• Strong organizational skills.
• Excellent communication skills.
• Proficient computer skills, including knowledge of Excel to create spreadsheets using formulas.
• Ability to read and comprehend instructions, correspondence, and memos; effectively present information in one-on-one and small group settings.
• Basic arithmetic skills: ability to add, subtract, multiply, and divide using whole numbers, common fractions, and decimals.
• Ability to apply common-sense understanding to carry out instructions furnished in written, oral, or diagram form.
• Ability to deal with problems involving several concrete variables in standardized situations.
• Must be able to perform each essential duty satisfactorily.
• Bilingual preferred.
