About the role

The Residential Case Manager provides clinical case management services to support and advance program goals through a high-quality, client-centered approach. They identify clients’ needs using a person-centered framework that addresses mental health symptoms as well as co-occurring disorders, and collaborate with clients to achieve the highest possible level of independence.

Responsibilities

  • Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
  • Work closely with the care team to align case management services with clients’ mental health treatment plans.
  • Conduct comprehensive assessments to identify client needs related to housing, education, employment, financial stability, healthcare, and community resources.
  • Develop, present, and implement individualized care plans in collaboration with the care team, clients, and their families as appropriate.
  • Provide support counseling for mental health issues, substance use disorders, and medical conditions.
  • Provide ongoing monitoring and follow-up to ensure client needs are met and progress is achieved; implement emergency protocols when required.
  • Assist clients with daily activities and life skills through modeling and education to promote independence, including medication management and budgeting.
  • Supervise clients self-administering medications while using the 6 Rights of Medication Self-Administration.
  • Assist and monitor group home clients with medical interventions and hygiene needs; conduct daily room/unit inspections.
  • Provide and maintain a home atmosphere; assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups.
  • Negotiate and schedule appointments for clients within The Center and with community partners (Social Security Supplemental Insurance, SNAP, Advocates Office, Medical Doctor, etc.).
  • Connect clients to community resources, including social services, transportation, and financial assistance programs.
  • Serve as a liaison between clients and community agencies, advocating for client needs and facilitating access to services.
  • Maintain and promote family contacts and relationships; provide education and guidance to clients and families on navigating systems such as healthcare, education, and social services.
  • Identify, assess, and respond to clinical emergencies and crisis situations, including suicide risk and danger to others; perform crisis intervention and risk management as needed.
  • Maintain accurate, detailed, and timely records of all client interactions and services provided, including clinical documentation as required.
  • Prepare reports and documentation for audits and program evaluation; ensure compliance with agency policies, ethical standards, and regulatory requirements.
  • Attend team meetings and case reviews to share updates and collaboratively address client challenges.
  • Develop professional relationships with Day Program staff; foster cooperative relationships with staff, clients, and community partners.
  • Transport clients safely to appointments and activities as needed.
  • Enter Work Request Orders for facility-related issues as needed and communicate such to the department team.
  • Attend trainings and case presentations; serve on committees and participate in agency initiatives as assigned.
  • Perform other duties as assigned to support client goals and program needs.

Requirements

  • High School diploma or GED required.
  • Minimum 1 to 3 years of experience in a behavioral health setting; experience in residential facilities preferred.
  • Valid driver’s license, registration, current inspection, and insured automobile.
  • Strong knowledge of community resources and systems, including housing, education, employment, and healthcare.
  • Ability to build rapport and establish trust with diverse clients and community partners.
  • Cultural competence and commitment to providing equitable and inclusive services.
  • Excellent communication skills, both verbal and written, to collaborate with multidisciplinary teams.
  • Strong organizational, problem-solving, and time management skills.
  • Proficiency in documentation and electronic record-keeping systems, including electronic medical records.
  • Ability to travel within the community with reliable transportation.
  • Able to work flexible hours, including evenings, weekends, holidays, and during inclement weather, as well as cover the program for 24-hour coverage when unforeseen circumstances arise.
  • Work environment is a residential setting for adults with severe psychiatric illness requiring 24/7 monitoring.
  • Bilingual preferred; for positions designated as bilingual, proficiency in the required language is mandatory, and the language assessment must be passed within 30 days of starting the role.
