About the role
The Housing Navigator, as a member of the St. John's Homeless Services team, works closely with case managers and patients to complete applications and prospective property owners and managers to identify permanent housing opportunities for individuals and families experiencing homelessness who are enrolled in St. John's ICMS, medical, dental, SUD, and/or Behavioral Health services.
Responsibilities
- Provide housing navigation services to ensure clients are supported in gathering documentation needed for housing screening
- Assist with obtaining ID, homeless history, documentation of disability and other required documents for housing
- Provide client advocacy, case management, benefit establishment, linkage to mental health/substance abuse services, linkage to stable housing and all other supportive services as needed
- Use evidence-based practice models such as Trauma Informed Care and Motivational Interviewing, along with Harm Reduction and Housing First principles
Qualifications
- Works under the direction of the RS&CAPS Director

This position is contract-funded with DHS. The contract is scheduled to expire on 6/30/2022. The contract may or may not be renewed again after that date. Your employment beyond the contract expiration date is not guaranteed. Should the contract not be renewed, you are eligible and encouraged to apply for another position within the agency.
