Job Summary

The Housing Manager provides direct housing case management and supportive services to individuals and families participating in the Housing Vulnerability Program. This position is responsible for conducting housing assessments, developing individualized housing plans, coordinating referrals, maintaining comprehensive case records, and facilitating access to community resources. The Housing Manager works collaboratively with landlords, service providers, funding agencies, and clients to promote housing stability, reduce barriers to care, and improve overall client outcomes. The position requires regular client engagement, documentation of services, housing advocacy, and ongoing monitoring to support long-term housing retention and self-sufficiency.

About the Role

This position is responsible for conducting housing assessments, developing individualized housing plans based on client needs, goals, and eligibility requirements. Provide housing case management services to individuals and families experiencing housing instability, homelessness, or other barriers to obtaining and maintaining safe housing. Assist clients with securing and maintaining permanent housing through advocacy, referrals, landlord engagement, and coordination of supportive services.

Responsibilities

  • Develop and maintain collaborative relationships with landlords, property managers, housing providers, community agencies, healthcare organizations, and other stakeholders to expand housing opportunities for clients.
  • Coordinate and facilitate referrals to community resources, including healthcare, behavioral health, substance use treatment, employment services, financial assistance, transportation, and other supportive services.
  • Maintain accurate, timely, and comprehensive documentation of all client contacts, assessments, service plans, referrals, progress notes, and housing-related activities in accordance with program, funding, and regulatory requirements.
  • Monitor client progress toward housing stability goals and provide ongoing support to address barriers that may impact housing retention and overall well-being.
  • Conduct home visits, outreach activities, apartment inspections, and community-based meetings as required to assess client needs and monitor housing placements.
  • Absorb and understand lease obligations, including rent payments, housekeeping standards, safety requirements, and landlord expectations.
  • Maintain current knowledge of housing resources, supportive housing programs, rental assistance programs, HOPWA, Permanent Supportive Housing (PSH), Section 8, and other available community resources.
  • Complete all required reports, data entry, program tracking, and outcome measures in a timely and accurate manner.
  • Participate in case conferences, interdisciplinary team meetings, staff meetings, trainings, audits, and quality improvement activities as assigned.
  • Ensure compliance with all organizational policies, grant requirements, confidentiality standards, and applicable federal, state, and local regulations.
  • Advocate for clients and promote equitable access to housing and supportive services while utilizing a trauma-informed and client-centered approach.
  • Perform other duties as assigned to support the goals and objectives of the Housing Vulnerability Program.

Qualifications

  • Associate degree in Social Work, Human Services, Psychology, Public Health, or a related field required; Bachelor's degree preferred.
  • Minimum of one (1) year of experience in case management, housing services, care coordination, or social services.
  • Experience working with underserved, vulnerable, or at-risk populations preferred.
  • Valid Illinois Driver's License and reliable transportation.
  • Strong communication, organizational, documentation, and problem-solving skills.
  • Proficiency in Microsoft Office and electronic documentation systems.
  • Ability to work independently, manage multiple priorities, and maintain confidentiality.

SMART Goals

  • Housing Stability: Assist at least 90% of assigned clients in developing and maintaining an individualized housing plan within 30 days of enrollment.
  • Documentation Compliance: Maintain 100% timely and accurate completion of assessments, case notes, service plans, and required program documentation.
  • Client Engagement & Resource Coordination: Ensure at least 85% of assigned clients receive appropriate referrals and supportive services that promote housing stability and self-sufficiency.

Working Conditions and Physical Requirements

  • Primarily community-based and office-based work environment.
  • Frequent travel within the service area for client visits, housing inspections, and community engagement activities.
  • Regular interaction with individuals experiencing housing instability, homelessness, HIV/AIDS, behavioral health challenges, substance use disorders, and other complex social service needs.
  • May require occasional evening or flexible hours to meet program and client needs.

Compensation & Benefits

Salary: $50,000
