About the role
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.
Responsibilities
- Coordinate and oversee the implementation of the SSVF (Supportive Services for Veteran Families) program.
- Provide a rapid re-housing program using a Housing First model to obtain and retain safe, long-term housing.
- Provide case management for self-sufficiency.
- Conduct landlord, community, and housing stakeholder outreach and collaboration.
- Coordinate direct financial assistance for veterans and agreements with landlord partners.
- Perform coordination duties and provide some direct service and mobile advocacy.
Requirements
- High school diploma or GED required.
- Associate Degree (or higher) in a field related to behavioral health preferred but not required.
- 1–3 years of full-time healthcare-related work experience where prior experience provides exposure to behavioral and medical crisis situations preferred but not required; or equivalent combination of education and experience.
- Current Arizona driver’s license.
- Clear 39-month motor vehicle record.
- Behavioral Health Technician (BHT) credential in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
- Arizona fingerprint clearance card, site specific.
