Housing Case Manager I (Bilingual English Spanish)

Under the supervision of the HHPS Associate Director, Program Manager, or Quality & Training Manager, the Housing Case Manager I serves households at the highest risk of becoming homeless or currently experiencing homelessness. The role ensures successful connections to community resources such as legal services, mediation, public benefits, and/or behavioral health services from program enrollment to exit, while also handling administrative financial and record-keeping functions.
Responsibilities
- Conduct initial problem-solving conversations with each household seeking services.
- Identify creative solutions to prevent or quickly resolve homelessness.
- Develop and implement Housing Stability Plans in collaboration with households.
- Follow up and refer non-eligible households to Access Points or other financial, housing, or resource assistance as appropriate.
- Refer and connect households in need of mediation, legal services, or advocacy to relevant services.
- Provide Housing Focused Case Management and make connections to other programs and organizations.
- Provide translation and interpreter services as needed.
- Complete a Housing Stability Plan, including a basic household budget, for all households receiving services.
- Ensure households remain stably housed from program enrollment to program exit.
- Ensure households increase their monthly income from program enrollment to program exit.
- Provide budgeting and money management assistance or connections to related services.
- Facilitate appropriate external referrals and serve as a liaison to support services consistent with client needs.
- Input data into required systems for reporting and prepare monthly, quarterly, and annual narrative program reports.
- Maintain confidential eligibility documentation in appropriate systems and hard copy.
- Conduct outreach, engagement, and follow-up to assure continuity of housing services.
- Monitor cases to ensure participants remain stably housed, increase income, and connect with community resources.
- Gather participant feedback using a written annual survey to assess service effectiveness.
- Provide monthly, quarterly, and annual reports summarizing contract activities, accomplishments, and challenges.
- Organize and provide relevant workshops.
- Enter required metrics in databases.
- Provide exceptional customer service and represent the organization professionally.
- Maintain personal safety, use PPE, and follow company safety policies.
- Report safety issues, accidents, or injuries to management promptly.
- Promote the health and safety of participants and follow emergency procedures.
- Interact positively with team members and facilitate positive change.
- Partner with supervisors to address department issues or concerns.
Requirements
- Bachelor’s Degree in Education, Human Services, Social Work, Psychology, or related field, or equivalent work experience.
- 6 months of experience working with clients from low-income or impacted backgrounds, including those experiencing homelessness, domestic violence, or COVID recovery.
- SF ERAP experience is a plus.
- Ability to provide services to a diverse population and work independently.
- Current active driver’s license preferred.
- Background check clearance required.
- Bilingual in English and Spanish preferred.

Schedule
- 40 hours per week, Monday through Friday.
- Must be available periodically on evenings and weekends.
Physical Demands
- Sit approximately 80% of the time, walk 10%, and stand 10%.
- Frequently use hands and fingers to type, handle, or operate objects.
- Occasionally lift and/or move up to 20 pounds.
- Occasionally reach with hands and arms.
- Frequently required to speak and listen.
- Specific vision abilities include close, distance, color, peripheral, depth perception, and ability to adjust focus.
Pay
$33.00 - $34.00 hourly
