Housing Case Manager
West CAP is an innovative, progressive organization dedicated to creating Strong, Secure Families, and Stable Communities since 1965. We are expanding our Homeless Intervention team by hiring a full-time Housing Case Manager.
About the role
This position will assist people experiencing homelessness or at risk of homelessness by providing case management to individuals and families. Responsibilities include assessment, individualized service plan development, support, advocacy, and referral.
This position may be based out of our Glenwood City or Menomonie locations with regular work-at-home options after training. Our team is collaborative, passionate, and focused on serving the needs of our participants with dignity and respect. We maintain a trauma-informed environment that supports each employee.
Requirements
- Associate degree in social work or related field; bachelor’s degree preferred
- Minimum of 2 years’ social service experience working with people experiencing poverty, mental illness, and/or substance use disorders
- Demonstrated experience with crisis intervention and case management
- Knowledge of community resources & social service systems
- Excellent written and oral communication skills
- Computer skills sufficient for data gathering, documentation, and reporting
- Valid driver’s license, reliable transportation, and acceptable driving record

Pay
Starting annual salary: $51,000 to $56,000
