Housing Case Manager

About the role

The Housing Case Manager is responsible for determining service plan goals, targets, and action steps for clients.

Responsibilities

• Meet with clients in-person or virtually to conduct an intake assessment that includes collection of information including demographic, financial, and social services information
• Document all assessments accurately and ensure the payer approves the services
• Provide case management of pre-tenancy services and tenancy-sustaining services
• Identify home modification needs

Requirements

• Associate's degree in human services or related field
• 1 year of experience providing case management services focused on pre-tenancy and tenancy-sustaining services
• Reliable telephone
• Empathetic to the needs of a diverse socio-economic population
• Strong interpersonal skills
• Knowledge of community resources, state and federal benefits and services
• Strong project management skills with the ability to administer multiple cases simultaneously and prioritize workload
• Basic math and budgeting skills
• Ability to work effectively as a team member with other members of program and agency leaders
• Demonstrated prior success in case management functions or eligibility determinations for community-based social service organizations
• Problem-solving abilities
• Ability to communicate and collaborate with health providers and community partners
• Strong internet, database and computer skills
• Good documentation skills
• Ability to read and write Spanish preferred

Physical Requirements

• Prolonged periods of sitting at a desk and working on a computer
• Must be able to lift up to 15 pounds at times

Pay

$24.52 - $33.17 per hour
