About the role
The Hospital Teacher/School Liaison at Children’s Healthcare provides educational support services to children who will be absent from school for an extended period due to injury, illness, and/or treatment. This role works closely with the patient's school team to ensure continuity of education.
Responsibilities
- Introduce Children’s Educational Support Services to patients and families identified through Epic consults, staff referrals, and by hospital teachers.
- Contact patient’s school team to advocate for patient and develop a modified school plan using patient’s school curriculum.
- Facilitate ongoing communication and sharing of patient information with family, school team, and Children’s medical team.
- Advocate for and participate in creation of Individualized Education Plan (IEP), 504 plan and/or Homebound Instruction (HBI) to support patient’s educational and medical needs.
- Facilitate learning by planning, creating, and assisting patients with schoolwork, returning completed assignments, and proctoring tests and quizzes.
- Facilitate connections to classmates and teachers to support patient’s social-emotional health.
- Maintain ongoing and current documentation of educational support services provided by Children’s, school team interactions and supports, and patient’s academic progress in the electronic medical record.
Qualifications
- Bachelor's Degree in Education from an accredited college or university required.
- Minimum 5 years of classroom teaching experience required.
- Basic Cardiac Life Support (BLS) required.
- A current Nebraska teaching certificate in Elementary Education (K-6), or Secondary Education (7-12) or the ability to obtain a Nebraska teaching certificate upon hire required.
Skills And Abilities
- Ability to conduct academic assessments, provide education elementary and secondary students.
- Ability to assess student learning needs and provide appropriate teaching methods for each student.
- Computer and Intranet skills.
- Communicate effectively when working with staff, students, parents and outside agencies.
