Key Responsibilities
- Collaborate with hospital case managers, social workers, nurses, physicians, and discharge planners to coordinate patient transitions to home care
- Facilitate timely delivery, setup, and support of respiratory and DME equipment upon patient discharge
- Educate patients and caregivers on proper equipment usage, safety practices, and maintenance
- Verify referrals, insurance information, and required documentation to ensure efficient service delivery
- Maintain accurate records, activity logs, and patient documentation
- Aid with urgent equipment needs and help resolve service-related issues as they arise
- Develop and maintain strong relationships with healthcare providers and referral sources
- Serve as a trusted resource for discharge planning teams regarding home healthcare equipment and services
- Participate in educational programs, in-service training, and community outreach initiatives
- Attend industry events, networking opportunities, and healthcare-related functions as needed
- Travel locally to support equipment setup, patient needs, and provider relationships
Qualifications
- High School Diploma or GED
- Valid driver's license and acceptable driving record
- Experience in healthcare, home health, medical equipment, patient services, case management, discharge planning, or related healthcare environments
- Knowledge of respiratory therapy equipment and Durable Medical Equipment (DME)
- Familiarity with insurance verification, healthcare documentation, or medical billing processes
- Understanding of medical terminology
- Three or more years of related healthcare or patient service experience
