Hospital Access Navigator

About the role

Hospice of Marion County, a member of Empath Health is seeking a Hospital Access Navigator to join our Admissions team. This full-time, leadership role offers an opportunity to make a meaningful difference in patient access and care coordination across our hospital partners. Since 1983, Hospice of Marion County has been honored to serve families across Ocala and Marion County with compassionate, expert end-of-life care. Responsibilities

• Provide daily leadership, oversight, and support to the Access Team staff within the Admission–Care Navigation Department.
• Serve as the primary liaison to hospitals, communicating Empath Health’s Mission, Vision, and values to partners and stakeholders.
• Ensure compliance with all state and federal regulations, as well as standards of care established by accrediting agencies.
• Coordinate and evaluate patient access and care navigation processes to promote quality, efficiency, and continuity of care.
• Monitor performance and support staff development through coaching, training, and performance feedback.

Requirements

• A.S. degree required; Bachelor’s degree (B.A.) from a four‑year college or university and Master’s degree preferred.
• Two years of hospice experience with at least two years of management experience preferred.
