Hospice Weekend Visit Nurse

Position Summary

This position will work in collaboration with triage nursing to make skilled nursing visits to patients/families needing after-hours / overnight support. Friday, Saturday and Sunday shifts available.

Qualifications

• Graduate of accredited school of nursing
• Current state of Minnesota RN or LPN licensure
• Three years recent nursing experience in acute, long-term care or home care/hospice/public health
• Patient care planning and patient teaching skills
• Written and verbal communication skills
• Critical thinker
• Current Minnesota driver’s license, insurance and daily access to reliable vehicle
• Computer literacy

Job Duties

• Displays an attitude of courtesy and respect for all prospective patients, families, staff and referral sources
• Works closely with triage nurse and makes face to face visits as directed
• Functions as a team member with other staff in the performance of duties
• Communicate significant findings, problems, patients’ needs, changes in condition or environment to the physician, clinical manager and other members of the hospice care team and obtains physician orders as required
• Sets and accepts feedback regarding the performance of job duties
• Awares of patient confidentiality and adheres to HIPAA regulations
• Displays an understanding of vulnerable adult issues
• Displays an understanding of hospice patients’ rights
• Exhibits sound mature judgment
• Excellent patient care delivery
