Schedule

  • NOC Shift | Monday-Friday | 4:00 AM - 12:30 PM
  • Includes every other weekend rotation and 3 holidays per year

Responsibilities

  • Perform telephone triage and patient assessment (75% of time):
    • Respond to patient and family calls in a timely manner
    • Effectively triage patient/family needs via phone and assess the need for a home visit
    • Interact positively and effectively with patients, families, collaborating agencies, and healthcare providers, focusing on excellent customer service
    • Utilize data collected from patients, families, and interdisciplinary team members
    • Apply knowledge of uncomplicated grief and the dying process in assessments and interventions
    • Articulate, identify, and report signs and symptoms of client abuse, neglect, suicide ideation, or violence
    • Assess the need for individualized patient and family education
  • Develop and document effective acute care planning (20% of time):
    • Assess pain on an ongoing basis
    • Collaborate with patients, physicians, and interdisciplinary team members
    • Use pain management skills to aggressively intervene during a pain crisis
  • Miscellaneous (5% of time):
    • Participate in mandatory educational requirements
    • Precept new staff

Requirements

  • ADN required; BSN preferred
  • Current, unrestricted Registered Nursing License in the state of employment
  • One year of nursing experience preferred
  • Must be at least 18 years of age
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life processes
  • Ability to maintain professional boundaries with patients, families, and co-workers
  • Ability to create and follow plans of care, and patient, family, and clinical staff directions
  • Basic Microsoft Office skills
  • Intermediate electronic health records skills; Epic preferred
  • Ability to work effectively and cooperatively with others
  • Strong verbal and written communication skills
  • Flexibility in time and task to meet organizational needs
  • Ability to maintain confidentiality regarding patients, families, caregivers, staff, donors, and organizational intellectual property
  • Ability to use proper body mechanics, safe patient handling, and standard precautions
