Hospice Resource RN - NOC Shift (North Madison Area)

Location: Sun Prairie/North Madison, WI area

About the role

0.5 FTE (20 hours/week) on the NOC shift, primarily weekdays from 0000–0830. Includes on-call rotation: one NOC shift per month, every other weekend, and three holidays per year. Sign-on bonus: $10,000.

Responsibilities

  • Patient and Family Assessments
    • Assess patients and families for physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs as outlined in the plan of care (POC).
    • Collect and document data from patients, families, interdisciplinary team members, and other healthcare professionals.
    • Identify, interpret, and document assessment findings, including significant changes in condition or unusual incidents (e.g., abuse/neglect, suicidal intent, harmful coping mechanisms).
    • Collaborate with the interdisciplinary team (IDT) and primary care provider to create a Hospice Evaluation Summary (HES) for eligibility determination.
  • Patient Visits
    • Respond to patient and family needs, including changes in condition or requests for acute intervention.
    • Provide interventions, treatments, and pain management tailored to patient and family needs.
    • Evaluate and adjust pain management or other interventions for effectiveness.
    • Educate patients and families on pain/symptom management, end-of-life care, and related topics, adjusting education as needed.
  • Plan of Care
    • Formulate, implement, and revise individualized POCs in collaboration with the IDT, patient, and family, including goals, pain management, interventions, education, and discharge plans.
    • Develop achievable outcome measures and establish care priorities.
    • Document interventions, outcomes, and changes in patient condition in the electronic health record (EHR).
    • Delegate visits to appropriate disciplines and manage visit schedules.
    • Oversee the work of Certified Nursing Assistants (CNAs) and Licensed Practical Nurses (LPNs).
  • Miscellaneous
    • Participate in meetings to plan and evaluate patient/family services.
    • Precept new staff and students.

Requirements

  • Current, unrestricted Registered Nursing License in the state of employment.
  • One year of nursing experience (preferred).
  • Valid driver’s license, auto insurance, and access to a reliable vehicle.
  • Must be at least 18 years of age.
  • Basic Microsoft Office skills.
  • Intermediate electronic health records (EHR) skills.
  • Ability to use proper body mechanics, safe patient handling, and standard precautions.
