About the role
Care Initiatives Hospice is seeking a PRN Hospice Spiritual Care Coordinator to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities.
Responsibilities
- Arrange for counseling or provide counseling to meet the spiritual needs of patients/families/caregivers in a manner consistent with their beliefs and desires.
- Provide comprehensive assessment of the patient, family, and caregiver’s spiritual needs.
- Complete bereavement risk assessments of the patient/family/caregiver and determine the bereavement needs.
- Identify appropriate spiritual and bereavement interventions and goals for the patients care plan.
- Ensure the services that are provided are in accordance with the patient’s acceptance of these services and the patient’s care plan.
Requirements
- Master of Divinity Degree from an accredited seminary; and ordination, if the particular religious group has the practice of ordination. If the religion does not practice ordination, but practices licensure, then licensure in lieu of ordination is acceptable; and endorsed and in good standing from the ecclesiastical governing body.
- OR Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education and is supervised by an MSW;
- OR Baccalaureate degree in psychology, sociology, counseling or other field related to pastoral care, grief services or counseling.
