Primary Responsibilities
- Conduct comprehensive psychosocial assessments to identify patient and family needs
- Provide supportive counseling addressing emotional, psychological, and grief-related concerns
- Connect patients and families with community resources and support services
- Collaborate with the interdisciplinary team to develop individualized psychosocial care plans
- Educate patients and caregivers about hospice services, advance care planning, and coping strategies
- Offer crisis intervention and emotional support during urgent situations
- Maintain accurate, timely documentation in accordance with state, federal, and agency standards
- Advocate for patient rights, preferences, and culturally sensitive care
- Participate in care team meetings and contribute to coordinated care strategies
- Stay informed on best practices in hospice care through ongoing education
Requirements
- Master’s degree in social work (MSW) from an accredited program
- Current Washington State LSWAIC or LICSW license required
- Active Basic Life Support (BLS) certification
- Valid driver’s license and reliable transportation
- Experience in hospice, palliative care, or related healthcare setting is strongly preferred
- Excellent interpersonal and communication skills
- Compassionate, patient-centered approach
- Able to work collaboratively in an interdisciplinary environment
- Strong organizational and time management skills
- Proficiency in electronic health records (EHR) and standard healthcare technologies
