Hospice Social Worker

About the role

Social Worker (Hospice) PRN – Service Area for Coverage: North Houston. The role supports the mission and Core Values by improving or maintaining each patient’s ability to control everyday physical, mental, emotional, spiritual, and psycho‑social needs.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the social services program of the hospice patient.
• Interview resident and family during admission process to complete a social history and assessment of the patient to identify problem areas and develop rapport.
• Explain and obtain Advanced Directives (Directive to Physician, Medical Power of Attorney) with residents and families.
• Work with Nursing department to ensure accurate Do Not Resuscitate orders are obtained for patients.
• Develop and implement policies and procedures for identifying the medically‑related social and emotional needs of the patient.
• Apply Social Work Theory, knowledge, methods, ethics, and the professional use of self to restore or enhance social, psychosocial, or biopsychosocial functioning of patients, under qualified supervision or with the appropriate license.
• Other duties as assigned.

Requirements

• Bachelor’s or Master’s degree in Social Work.
• Licensed Social Worker in the State of Texas.
• Minimum of two (2) years of social work experience in a supervisory capacity in hospice or related medical home health field.
• Outstanding organizational skills and ability to deal tactfully with associates, patients, family members, visitors, government agencies/personnel, and the public.
• Proficiency with computer systems (Word, Excel, Electronic Health Record system, etc.).
• Good standing with Federal and State Government.
