Hospice Social Worker

Qualifications

• Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
• Current Social Work licensed in the state(s) of practice.
• Four (4) licensure levels: Licensed Social Work (LSW), Licensed Graduate Social Worker (LGSW), Licensed Independent Social Worker (LISW), Licensed Independent Clinical Social Worker (LICSW).
• Ability to pass background, consumer report, and drug test.

Core Responsibilities

• Meet with patients and their loved ones.
• Collaborate closely with the clinical team.
• Connect families with appropriate community resources and services.
• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
• Maintain reliable transportation, valid driver’s license, and proof of auto insurance.

Pay

• The starting salary range for this role is $54,000-$85,000 annual base salary.
