HOSPICE SOCIAL WORKER

Job Duties

• Visits patients wherever they reside, including private homes, skilled nursing facilities, assisted living facilities, and hospitals.
• Completes a psychosocial assessment on each assigned patient in a timely manner and documents in the electronic medical record.
• Utilizes community resources for patients, such as in-home services, Meals on Wheels, etc.
• Aids the patient, family, and caregiver in coping with issues related to death and dying and provides grief support as needed.
• Provides information about hospice services and completes hospice admission paperwork.

Qualifications

• Graduated from a School of Social Work approved by the Council of Social Work Education with a Master's or Bachelor's Degree in Social Work.
• Other related degrees/areas of study are not accepted due to state regulations.

Work hours are primarily Monday through Friday, 8am to 5pm, with flexibility required for scheduling and hours. Hourly rate, mileage reimbursement, and computer skills for charting/EMR, basic use of Word, Excel, Email, and smart phone for mileage apps are provided. Compensation is based on years of experience. A loving and compassionate heart is a must.
