About the role
The Social Worker at Caring Hospice provides quality hospice care, end-of-life, educational, and grief support services.
Responsibilities
- Conduct hospice assessments and patient visits
- Provide comprehensive hospice care
- Work effectively on an interdisciplinary team
- Make funeral arrangements
- Plan patient care
- Access and utilize community resources
- Discharge planning
- Provide grief counseling and support
- Foster open communication with patients, families, facility partners, staff, and other healthcare providers
Requirements
- Must be a Licensed Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) in the state of MA
- Minimum of 2 years of hospice experience preferred in hospice or oncology
- Previous experience in conducting comprehensive assessments
- Ability to work effectively on an interdisciplinary team
- Prior experience making funeral arrangements; patient care planning, accessing community resources and discharge planning
- Ability to provide grief counseling/support
- Ability to foster open communication with patients, patient’s families, facility partners, staff and other healthcare providers
Qualifications
- Reliable transportation
- Ability to travel to where the patient resides
