Hospice RN - Weekends

Role: Afterhours Registered Nurse (RN) providing comprehensive hospice care outside regular hours, including nights and weekends. Responsibilities Triaging patient needs via phone until in-person visits can be conducted.Responding promptly to crisis calls from patients and families afterhours.Performing initial nursing evaluations to determine hospice eligibility within 48 hours of referral.Conducting afterhours visits such as admissions, death, recertification, and routine visits.Assessing and reassessing patient and family needs across physical, psychosocial, emotional, and environmental domains at least every 15 days.Documenting problems, goals, interventions, and responses to care.Providing care in diverse home environments, regardless of patient background. Qualifications & Skills Current unrestricted RN license in the practice state.Valid driver’s license, insurance, and access to reliable transportation or public transit.Preferred: CPR certification (can be obtained within 90 days of hire), minimum one year of clinical experience, or three years as an LPN if applicable.
