Hospice RN - Weekend PRN

Key Responsibilities

• Perform initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
• Assess and reassess patient’s physical, psychosocial, and environmental needs regularly.
• Initiate and coordinate care plans, maintaining accurate clinical documentation.
• Document patient problems, goals, interventions, and responses to care.
• Collaborate with patients, families, physicians, and interdisciplinary team members for daily care.

Qualifications & Skills

• Active, unrestricted RN license in the practice state.
• At least one year of clinical experience.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and insurance or access to public transit.
