Hospice Registered Nurse - Weekend PRN

Role Overview

As a Per Diem Registered Nurse Case Manager, you will be responsible for delivering comprehensive hospice care by assessing patient needs, developing and coordinating individualized care plans, and ensuring the delivery of quality services. You will evaluate patient eligibility, re-assess needs regularly, document clinical progress, and collaborate with patients, families, physicians, and the interdisciplinary team to provide compassionate care.

Key Responsibilities

• Conduct initial nursing evaluations within 48 hours of referral to determine hospice eligibility
• Identify and reassess physical, psychosocial, and environmental needs every 15 days
• Initiate and coordinate care plans, maintaining accurate clinical documentation
• Document patient problems, goals, interventions, and responses to care
• Collaborate with the care team to provide holistic patient and family support

Qualifications & Skills

• Current unrestricted RN licensure in the practice state
• Minimum of 1 year of clinical experience
• Valid driver's license, insurance, and access to reliable transportation or public transit
• Preferred: CPR certification or ability to complete within 90 days

Compensation & Benefits

• Hourly pay ranges from $35.45 to $53.15, based on experience and location
