Hospice RN On Call

About the role

The On Call/After Hours RN position is dedicated to providing skilled nursing care to hospice and home health patients and their families after normal business hours. This role requires a strong commitment to excellent customer service and the ability to manage complex patient needs while adhering to regulatory standards.

Responsibilities

• Triage calls from patients and caregivers to ensure accurate evaluations and development of comprehensive care plans.
• Maintain timely home/facility visits to assess and adjust patient and family needs.
• Educate patients and families on various aspects of care, including symptom management and end-of-life care.
• Administer medications accurately and document all actions in accordance with company and regulatory standards.
• Communicate effectively with hospice and home health teams regarding patient status and care plan adjustments.
• Report high-risk or critical issues to supervisors and management in a timely manner.
• Collaborate with the hospice and home health teams to address the comprehensive needs of patients and their caregivers.
• Supervise and coordinate the work of hospice aides and home health aides to ensure adherence to care plans.

Requirements

• Current unencumbered RN license in the state of Ohio.
• Bachelor’s degree in Nursing preferred.
• Three years of experience in acute care, medical-surgical, oncology, or geriatric nursing.
• Experience in a Medicare-certified home health agency preferred.
• Proficiency with Windows, Microsoft Office, and the internet.
• Proficiency with electronic medical records (EMR) preferred.

Qualifications

• Education: Bachelor’s of Science in Nursing (BSN) preferred.
• Experience: Three years of experience in acute care, medical-surgical, oncology, or geriatric nursing required.
• Other Requirements: Valid driver's license, acceptable driving record, and insurability under professional liability and crime coverage policies.
