About the role
The On Call/After Hours RN position is dedicated to providing skilled nursing care to hospice and home health patients and their families after normal business hours. This role requires a strong commitment to excellent customer service and a deep understanding of hospice and palliative care standards.
Responsibilities
- Triage calls from patients and caregivers to ensure accurate evaluations and development of comprehensive care plans.
- Make timely home/facility visits to assess and adjust the care plan based on patient and family needs.
- Communicate with other healthcare providers and families regarding changes in the patient's condition and needs.
- Educate patients and families on physical, emotional, and psychosocial aspects of care, disease processes, symptom management, and signs of death.
- Administer medications accurately and document all actions in accordance with company standards and regulations.
- Complete hospice admission assessments, home health admission assessments (OASIS), and develop patient's initial plan of care.
- Attend Hospice Interdisciplinary Group meetings and communicate patient care information to the hospice team.
- Communicate patient care information to the home health team in a timely manner.
- Inform Clinical Supervisors, Administrators, and/or Executive Directors of any high-risk or critical issues.
- Document patient and family care and necessary changes in the plan of care to maintain accurate patient records.
- Maintain knowledge of and comply with regulatory agencies, accrediting bodies, third-party payers, and HIPAA standards.
- Collaborate with the hospice team, home health team, and physicians to address the comprehensive needs of patients and their caregivers and families.
- Supervise hospice aides, home health aides, and coordinate other disciplines to direct patient caregivers/family members in implementing necessary changes to the patient's plan of care.
Requirements
- Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
- Bachelor’s of Science in Nursing (BSN) preferred.
- Three years experience in acute care, medical-surgical, oncology, or geriatric nursing required.
- Experience in a Medicare-certified home health agency preferred.
- Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
- Proficiency with electronic medical records (EMR) preferred.
Qualifications
- Education: Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required. Bachelor’s of Science in Nursing (BSN) preferred.
- Experience: Three years experience in acute care, medical-surgical, oncology, or geriatric nursing required. Experience in a Medicare-certified home health agency preferred.
- Other Requirements: Must be able to read, write, speak, and understand the English language. Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
