Job Summary
Journey Hospice is seeking a Full-Time RN Case Manager to serve clients in Adams and Franklin Counties. This position offers a competitive salary and robust benefits.
Responsibilities
- Oversight of primary caseload of Hospice patients
- Provides skilled professional nursing care to patients, families, and caregivers
- Develops and implements Hospice Interdisciplinary Team (IDT) plan of care
- Teaches, supervises, and counsels patients, family members, and staff regarding nursing care and patient needs
- Works closely with other members of the IDT to meet patient and family care needs
Requirements
- Graduate of an accredited registered nursing program
- Bachelor’s degree in Nursing preferred
- PA RN Licensure by the PA State Board of Nursing in good standing
- Valid drivers license and reliable transportation insured in accordance with PA state requirements
