As a Hospice RN Case Manager, you will deliver patient care through assessment, planning, implementation, and evaluation, organizing and directing hospice services with an emphasis on community health education.

Responsibilities

• Deliver patient care using the nursing process of assessment, planning, implementation, and evaluation.
• Plan, organize, and direct hospice care services.
• Build community resources to meet the needs of individuals and families in their homes and communities.

Requirements

• Valid Indiana Registered Nurse License (RN).
• Previous hospice or long-term care experience preferred.
