Hospice RN Case Manager - Full-Time

Role Overview: Provides specialized nursing care and support to patients at the end of life, focusing on symptom management, pain control, and holistic comfort. As a case manager, coordinates all aspects of patient care, developing and implementing personalized plans in collaboration with a multidisciplinary team, patients, and families. Promotes culturally sensitive, patient-centered care while advocating for patient needs and ensuring seamless communication across care settings. Key responsibilities include assessing patient preferences and barriers, updating care plans, evaluating patient condition changes, and ensuring evidence-based practices to achieve quality outcomes. The role requires strong critical thinking, empathy, cultural competence, and collaboration skills. Qualifications include a Nursing Diploma or Associate’s degree, RN licensure, a valid driver’s license, and at least 2 years of nursing experience. Preferred qualifications include a Bachelor's degree in Nursing or Home Health and 2-5 years of relevant experience.
