Hospice RN Case Manager - Full Time

Role: Hospice Registered Nurse (Full Time) responsible for pain management and symptom control in hospice patients, collaborating with an interdisciplinary team, attending physicians, and the Hospice Medical Director. Key Duties Assess patient and family needs during home visits and document assessments, interventions, goals, and outcomes.Coordinate comprehensive hospice care, ensuring continuity across providers and facilities, under supervision.Participate in developing, reviewing, and revising patient/family care plans, communicating progress and challenges.Supervise hospice aides and licensed practical nurses, ensuring compliance and proper documentation.Facilitate care in long-term care facilities, collaborating with staff and attending care plan meetings.Provide ongoing patient monitoring for hospice eligibility and complete certification documentation.Engage in on-call duties after hours, weekends, and holidays.Educate patients and families about hospice concepts and care processes. Qualifications & Skills Active RN license in the state of application.Valid CPR certification.Preferred: minimum 1 year of hospice experience.
