Hospice RN Case Manager - Full-Time

Role Overview: This position involves providing comprehensive end-of-life care, supporting patients and their families with a focus on pain management, symptom control, and emotional and spiritual needs. The nurse evaluates patient preferences, develops personalized care plans, and collaborates with an interprofessional team to ensure quality, evidence-based care. Responsibilities include assessing patient conditions, communicating updates, and advocating for patient-centered services in a safe, respectful manner. The role requires coordination of care across diverse populations and proactive planning to optimize patient outcomes.
