As a Hospice RN Case Manager, you will cover the Fishers, Lawrence, Greenfield, and New Castle, IN area. Responsibilities

• Deliver patient care through the nursing process of assessment, planning, implementation, and evaluation.
• Plan, organize, and direct hospice care services.
• Build from community resources to meet the needs of individuals and families within their homes and communities.
• Provide community health education and experience.

Requirements

• Valid Indiana Registered Nurse License (RN).
• Previous hospice or long-term care experience preferred.
