About the role
The RN Case Manager at Medical City Dallas plays a pivotal role in patient care, coordinating the plan of care, managing the length of stay, and ensuring appropriate resource management. They also facilitate the progression and transition of care, aligning patient needs with available resources.
Responsibilities
- Performs a comprehensive assessment of psychosocial, medical, and discharge needs of patients/family
- Reassesses the patient’s clinical condition as needed
- Develops strategies to mitigate patient readmission risks
- Collaborates with the multidisciplinary health care team to facilitate a successful care transition
- Ensures post-acute medical needs and level of care are appropriate
- Timely referral to Social Services for psychosocial determinants of health
- Involves patient, family/responsible/significant others in identifying and clarifying needs and expectations
- Evaluates progression of care using evidence-based tools and approved criteria
- Makes appropriate referrals to third party payer, disease and case management programs
- Facilitates patient throughput with an ongoing focus on an effective care transition
- Documents professional recommendations, discharge plan, care coordination interventions, and case management activities
- Acts as a liaison between various stakeholders
- Directs activities to identify and provide for the needs of the under-resourced patient population
Requirements
- Registered Nurse with current TX state license
- Associate Degree in Nursing or Nursing diploma
- 3+ years of clinical hospital nursing experience OR 2+ years experience in case management
- Certification in case management preferred
- InterQual experience preferred
Qualifications
- Bachelor's Degree in Nursing preferred
- Knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives
- Ability to advocate for patient's rights, needs, and values
