About the role
The Department 39109 Hospice North Region is seeking a Full-time Benefits Eligible position with a sign-on bonus opportunity. The role is located in Chicago Areas/zip Codes 60601-60651 and secondary assignments in neighboring areas.
Responsibilities
- Identify and coordinate patient/family care in hospice settings.
- Provide comfort care at end of life and maximize quality of life for patients and families.
- Educate patients and families on disease processes, management, and hospice philosophies.
- Update primary physician as needed and document in electronic health record.
- Review and update patient care plans and collaborate with interdisciplinary team.
- Administer medication and treatments, supervise paraprofessionals, and facilitate discharge planning.
- Conduct OASIS and hospice assessments, ensure skilled nursing/homebound criteria, and perform patient positioning.
- Mentor new staff and communicate patient needs to healthcare providers.
Requirements
- Registered Nurse license and BLS certification.
- 1-2 years of nursing experience in home health, hospice, or related fields.
- Knowledge of principles of deformity prevention, human behavior, and stress management.
- Excellent communication, interpersonal, decision-making, and organizational skills.
- Physical ability to lift up to 50 lbs. and perform various patient care tasks.

Pay
$38.20 - $57.30 per hour.
Schedule
Standard schedule: Monday–Friday, 8:00 AM–5:00 PM. Flexibility and on-call requirements are discussed with the manager or team lead.
