About the role
Provide compassionate care coordination for hospice patients in their homes. Collaborate with interdisciplinary team members to ensure comprehensive patient care plans.
Responsibilities
- Coordinate care plans with patients, families, and other healthcare providers.
- Assess patient needs and provide necessary support services.
- Ensure compliance with hospice policies and regulations.
- Communicate effectively with patients, families, and healthcare teams.
Requirements
- Bachelor’s degree in Nursing or related field.
- Current RN license in good standing.
- Experience in hospice care or home health preferred.
- Strong interpersonal and communication skills.
Qualifications
- Excellent organizational and time management skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite.
