About the Role

As an RN Case Manager, you will play a critical role in making our patients' final days, weeks, and months as comfortable as possible while working closely with the company's owners and contributing to an entrepreneurial spirit.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands‑on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.

Qualifications

• Licensed as a Registered Nurse (RN) in the state of employment.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid driver’s license and reliable vehicle with current insurance.
