Hospice RN Case Manager

About the Role

Seeking a Hospice Registered Nurse Case Manager to provide compassionate end-of-life care in patients' homes or facilities. The role involves assessing patient needs, developing and implementing personalized care plans, and collaborating with a multidisciplinary team to ensure quality, patient-centered care. The nurse will evaluate changes in patient conditions, communicate effectively with families and team members, and advocate for culturally sensitive support. This is a safety-sensitive position requiring a valid RN license, a valid driver’s license, and the ability to work in a safe, alert manner.

Responsibilities

• Assessing patient preferences, values, and barriers
• Creating and updating individualized care plans
• Promoting respect and empathy across diverse populations
• Monitoring patient progress and adjusting care as needed

Requirements

• Associate’s degree in Nursing or higher
• Valid RN license and driver’s license
• Less than 2 years of experience (preferred 2-5 years for advanced roles)
