Hospice RN Case Manager
Hospice RN Case Manager Position Summary
Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms.

Minimum Qualifications
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Georgia
- 2 years experience as a professional nurse in acute care, home care or ER experience required
- Home care experience preferred but not required
- EPIC experience preferred
