With an increasing census, we are now seeking a Full-Time RN Case Manager for our Garland Service Area, particularly in Collin County.

Responsibilities

• Perform and coordinate field visits with the nursing team and as needed by the patient's plan of care.
• Train and educate patients, family/caregivers, and staff on appropriate usage of medications, DME, and supplies.
• Ensure concerns are addressed and the patient is comfortable - delivering excellence in patient care and attention.
• Understand the hospice philosophy, pain management, and the needs of the terminally ill, communicating effectively with patients and families.
• Demonstrate excellent interpersonal and communication skills.
• Reflect Hope Health Care's agency values through actions and interactions.

Qualifications

• Completed degree in Nursing (either ADN or BSN).
• Valid RN license in Texas.
• At least one year of Hospice experience.
• Valid Texas Driver's License and proof of auto insurance.
• Reliable transportation.
• Valid BLS certification.

• Successful completion of a background screen, including employment verification.

Schedule

Day shift. Monday to Friday.
