About the Role
We are seeking a compassionate and experienced Registered Nurse (RN) to serve as a Hospice Case Manager in our Lakeville location.
- Supervise hospice aides
- Complete comprehensive assessments of patients and families to determine hospice needs
- Provide professional nursing care by utilizing all elements of the nursing process
- Assess and evaluate patient status, writing and initiating plans of care, and regularly re-evaluating patient and family needs
- Develop care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions
- Counsel patients and families in meeting nursing and related needs
- Provide health care instructions and assist patients with activities of daily living
- Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload
Weekly pay range of $42 to $52 an hour, depending on experience.
Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
- Current licensure in the State and CPR certification
- Bachelor's degree, with one (1) year of hospice care experience preferred
