Hospice RN Case Manager

About the role

The Registered Nurse (RN) is responsible for delivering patient care through the nursing process of assessment, planning, implementation, and evaluation. The RN plans, organizes, and directs hospice care services, with an emphasis on community health education and experience. The professional nurse builds from community resources to meet the needs of individuals and families within their homes and communities.

Responsibilities

• Deliver patient care using the nursing process of assessment, planning, implementation, and evaluation.
• Plan, organize, and direct hospice care services.
• Leverage community resources to address the needs of individuals and families in home and community settings.

Requirements

• Valid Indiana Registered Nurse License (RN).
• Previous hospice or long-term care experience preferred.
