Hospice RN Case Manager

Full-time RN Case Manager position in McHenry County (60014). Territory covers Monday–Friday, 8:30 a.m.–5:00 p.m. with on-call duty every 12th weekend (Saturday and Sunday).

Responsibilities

• Assume primary responsibility for a patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain and assess data from patient physical, psychological, social, and spiritual factors that may impact patient and family needs; initiate interventions by other members of the interdisciplinary team
• Initiate communication with attending physicians, interdisciplinary teams, and other agencies to coordinate optimal care
• Provide regular visits to re-assess patient needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports, and prepare care plans in a timely manner
• Supervise and document delivery of care by the Home Health Aide as required by federal, state, and company guidelines

Requirements

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Current CPR and First Aid certification
• Knowledgeable of quality assessment and assurance procedures
• Valid driver’s license and proof of insurance
