Hospice RN Case Manager

Requirements
- Valid GA RN license
- Valid driver’s license
- At least one year of experience as an RN
- Experience in hospice or home health (preferred)
Responsibilities
- Be a steady source of comfort and support to patients and families
- Help families understand disease progression, self-care, and nutrition
- Collaborate with the interdisciplinary hospice team to create and follow patient care plans
- Manage symptoms like a boss and advocate for patient comfort
- Maintain timely and accurate clinical documentation
Patients may be in their own home, an assisted living facility, or a skilled nursing home.
